Iowa, 2027 · Confirmed

Does any Iowa county have a marketplace PPO in 2027?

No. Zero of Iowa’s 99 counties have a PPO on the marketplace for 2027, and zero have a POS. That’s the published answer now, not a forecast. CMS published the plan year 2027 Plan Attributes and Service Area public use files on October 20, 2026. Joining them county by county returns 7 marketplace products for Iowa, meaning Individual (Marketplace) plans, also called ICHRA or CHOICE Arrangement plans, and every one of the 7 is an HMO or an EPO.

For 2026, 7 counties did have one: Clay, Dickinson, Emmet, Lyon, O’Brien, Osceola and Sioux. All 7 came from a single product sold by a single insurer. That product is absent from the published 2027 files, and that insurer’s only 2027 Iowa product is an EPO, covering “services received within the provider network.”

The same insurer made the identical change one state over, and said so plainly in that state’s filing: the product “previously operated as a PPO product through plan year 2026, but will convert to an EPO product for plan year 2027.” South Dakota keeps a marketplace PPO anyway, because a second insurer files one in all 66 counties. Iowa has no second insurer.

Nobody loses coverage. What goes away is the out-of-network benefit, in the only part of Iowa that had one. Everyone in those 7 counties moves onto a plan that pays for care inside the network only.

What it means if you employ people up there: if a CHOICE Arrangement was your route to keeping out-of-network coverage for staff in northwest Iowa, that route closes on January 1, 2027. If out-of-network access is the part your people care about, the answer moves to the group side, and the section below is where it lives.

Confirmed November 3, 2026. Until the files published, this page read the 2027 rate filings and said Iowa looked set to lose its only marketplace PPO. The published plan and service-area files settle it. The 2027 county counts above are published figures, not proposed ones, and the 2027 rates are approved.

If the only out-of-network plan your people could buy stopped being sold, how far would they travel to keep the doctor they have?

Sources: the CMS Plan Attributes public use file and Service Area public use file for plan year 2027, both published October 20, 2026, joined on issuer and service area and counted per county. The product conversion is described in the insurer’s 2027 Iowa individual rate filing, Part II written explanation dated June 5, 2026 and Part III actuarial memorandum dated June 4, 2026, and in its 2027 South Dakota individual Part III actuarial memorandum dated May 28, 2026, all published through the federal rate review system. Plan year 2026 county counts are from the CMS plan year 2026 public use files.

What it means for an Iowa employer

Your renewal is a number. What are you allowed to change about it?

An approved 10.3% sits below the national median of 14.8%. The market number isn’t your number, though. Two Iowa employers the same size can land 20% apart on the same filings.

The Iowa-specific read: group plans cost less per member per month here, so a CHOICE Arrangement isn’t the obvious first move. And for 2027 Iowa has no out-of-network coverage left on the marketplace, in any of the 99 counties. If out-of-network access is what your people care about, it now lives on the group side.

Where the money actually is at these rates: not the plan type, the design changes inside it. Sourcing 2,000+ medications at a $0 copay for the whole household. Unlimited virtual primary and behavioral care so a $0 visit replaces an urgent care trip. A supplemental layer that pays the employer back every pay period instead of adding a line item.

Those have landed at $2,000+ per employee per year in real client projects, with about $988 per employee per year in new cashflow on top of that and independent of it. About $100K per year for every 50 people. $0 upfront, billed out of savings, live in 30 to 45 days. Same broker, same doctors, same cards.

If your Iowa renewal came back flat instead of 10.3%, what would you put the difference into first?

Results vary by group size, plan design, and utilization.

Sounds concerning? You have options.

So what are the actual options in Iowa?

Iowa’s marketplace out-of-network coverage reached 7 counties for 2026 and reaches none for 2027. The published files settle that. It closes one route. It doesn’t close the rest, and the rest is where most of the money was anyway.

Here’s the part that isn’t anyone’s fault. The systems that surface plan options only surface what’s already inside them. The marketplace shows you what insurers filed for the marketplace. Your broker shows you the markets their appointments reach. Both are doing their job. Neither one can show you what sits outside it.

99% of the brokers we talk to have never had visibility to our programs. They sit outside their commissioned channels.

Whether it beats your group plan depends on the county mix in your census, not the state average.

Why the ranges are keyed to what you run today and not to a product name: that’s how they were measured. Every range above comes from completed client projects, grouped by the employer’s starting point. We don’t publish a savings percentage next to “level-funding” or “captive” as a category, because nobody measured a category. If you see a number like that anywhere, ask what it was measured on.

If one of these came back at the low end of its range for your Iowa group, which line in your budget would you want it to pay for first?

Ranges are what we have observed across completed engagements. They are not a guarantee, not a quote, and not an offer of coverage. Your result depends on group size, plan design, census, and utilization.

Questions Iowa employers ask

How much are health insurance rates going up in Iowa in 2027?

Regulators approved a median 10.3% increase on marketplace plans in Iowa and 6.1% on employer group plans for 2027. The national median across all individual market filings is 14.8%. Individual insurer approvals in Iowa range from 4.4% to 16.6%. Insurers had asked for 11.8%.

Is a PPO available in Iowa on the marketplace?

Not for 2027. Zero of Iowa’s 99 counties have a PPO on the marketplace for 2027, and zero have a POS. All 7 of Iowa’s 2027 marketplace products are HMOs or EPOs. For 2026, 7 counties did have a PPO, all from one product from one insurer, and that product is absent from the published 2027 files. Source: the CMS plan year 2027 Plan Attributes and Service Area public use files, published October 20, 2026.

What does a silver plan cost in Iowa?

The lowest cost silver plan for a 40-year-old non-smoker runs $518 per month in a typical Iowa county for 2026, ranging from $452 to $612 across counties, before any subsidy. The lowest cost PPO silver plan ran about $559 for 2026. For 2027 there is no marketplace PPO in Iowa to price.

Do I have to offer health insurance if I have fewer than 50 employees in Iowa?

Not under federal law. The ACA employer rule starts at 50 or more full-time employees, counting full-time equivalents and averaged over the prior year. Below that line it’s a choice rather than an obligation. Hawaii is the national exception: it requires coverage for anyone working 20 or more hours per week, with no headcount minimum at all. Some states run an employer assessment that helps fund public coverage, Massachusetts among them, which is a payroll charge rather than a requirement to offer a plan.

What are my options in Iowa with fewer than 50 employees, and what changes above 50?

Under 50, the designs that carry no headcount minimum are open to you: a CHOICE Arrangement (formerly ICHRA), QSEHRA, shelf-rated (flat-rate) guaranteed issue plans, and the supplemental layers that sit on top of whatever you already have. Level-funded generally opens up in the tens of lives. At 50 and above, self-funding, a captive and reference-based pricing come into range, and the ACA reporting obligations begin. A MEC-only plan is the cheapest way to clear the larger penalty, and worth comparing against a bronze-level design before you choose it. The usual surprise is how much is available below 50, not how little.

What’s my best option in Iowa?

It depends, and the honest version of that answer is a short list of variables rather than a product. Headcount, your workforce ages, whether anyone is mid-treatment, whether you want your claims data back, and what you can fund per employee. The range is wide: shelf-rated guaranteed issue plans, which price everyone the same regardless of age and can run alongside your existing plan rather than replacing it, bundles carrying $0 copays on prescriptions, telehealth and behavioral health, high-deductible designs with first-dollar coverage, and combinations in between. Do you have people in more than one state?

Filings, premiums and PPO availability all change at the state line, so a census that crosses one gets priced state by state rather than on a national average. These are the states nearest Iowa, each with the same figures.

Where these Iowa numbers come from

Rate changes are the midpoint of what regulators approved for each insurer in Iowa, from the CMS rate review service, pulled November 1, 2026. All 10 Iowa filings for 2027 carry a final determination. County premiums are the lowest cost plan per metal level for a 40-year-old non-smoker before any subsidy, from the CMS marketplace plan and rate files for plan year 2026, covering all 99 Iowa counties.

The marketplace versus group cost comparison uses the 2026 average premium per member per month from the insurers’ own filings on both sides, weighted by enrollment. It is an average across each plan’s enrolled mix, not a same-metal, same-plan-type quote. Plan-type availability joins the CMS Plan Attributes public use file to the Service Area public use file, both for plan year 2027 and both published October 20, 2026, counts every marketplace plan available in every Iowa county, and asks whether any of them is a PPO or POS, because the split that matters is whether a doctor outside the network is covered at all. Checked November 3, 2026. The plan year 2026 counts on this page come from the same two files for plan year 2026. Off-marketplace plans are counted separately, at state level, from the insurers’ 2026 rate filings.

Nothing here is a quote.
